Physician Pending-Members and Adjunct Pending-Members: A special code was emailed to you. Please enter that code into the field above, click ‘Submit’, and then select the appropriate registration type from those displayed. Contact the ISHRS HQ if you have not received your code.

Physician Non-Members: Please fax the appropriate registration verification to the ISHRS Headquarters and a special code will be emailed to you within 48-hours. This code will allow you to register for the meeting. If the aforementioned step has been completed and you have not received your special code within 48-hours, please call the ISHRS Headquarters.

Non-Physicians (Surgical Assistants, Office Managers, Office Staff, Clinic Directors or Spouse/Guests): (1) Your ISHRS employing physician* must register first. (2) Your ISHRS employing physician must fax or email a Letter of Attestation on your behalf. (3) Your special code will be emailed to your ISHRS employing physician. If your special code is not received within 48-hours, then please contact the ISHRS Headquarters.
